About the New Forms
School Claims Service Employee Benefits Division new online forms capability is username and password protected. School districts have the option to set up as many users as needed using the HRUser option. The online forms provide the ability to enter Administration Expense Reports, Fringe Benefits Enrollment Change Forms and COBRA Forms Enrollment. Districts also can print reports listing all their forms submitted on the Online Forms Administration login page.
Logging in
  1. From the SCS home page (www.schoolclaimsservice.com), go to Forms (top-page navigation)
  2. From the Forms page, go to Online Forms Administration -- Login Page
  3. At the login page enter your
bulletUsername*
bulletPassword*
bulletand then login
* Usernames and passwords were provided to each school district

To add all other users or to change or update information, from the left side of the page, choose to Edit My Profile. To add other users, select to Add New.

Role ID

bulletDistrict Admin = Business managers
bulletHR-user = All other school district employees who are to have access to these online forms
Once a user is set up, the online forms can be submitted to SCS Employee Benefits. After each form submission is completed, you will receive a confirmation number verifying that SCS has received your information. You also can use the report area to print any documents your district has submitted to SCS.
