For Employees: What to do if you are injured at work

  • Tell your supervisor immediately.
  • If you need to see a doctor, choose one from the list of Panel of Physicians and let your supervisor know.
  • Meet with your supervisor and complete the Accident Investigation and Injury Report.
  • Take the Physical Capabilities form to your doctor's appointment.
  • The doctor will treat your injury and bill the workers' compensation carrier for the services.
  • Never just call off work due to a work-related injury! The workers' compensation panel doctor must first evaluate your medical situation and make a determination as to what work you can and cannot do. The school district will then make a decision regarding modified duty for those who may be temporarily limited in the work they can perform.
